Report Writer now has the ability to add Forms to your report! 

Many users will find that they will not need to design new forms as to utilize this feature it requires a user who is comfortable with HTML5. 

NOTE: Izenda FORMS will override the normal reporting behavior within Izenda when a FORM is attached to a report; the FORM is what the engine renders. To use a standard Izenda report in tandem with a FORM, you must call the report into the HTML FORM itself. For more information on Forms please see the link to the Izenda Wiki below.
